MeCare is committed to protecting the privacy of your personal health information. As part of our business operations, we create and maintain records related to your treatment and services. We are legally obligated to maintain the confidentiality of your personal health information.

Breach Notifications: In the event of a breach involving your unsecured personal health information, we will provide written notice within 60 days. The notice will include a description of the breach, steps you can take to protect yourself, and our actions to address the breach.

Accounting of Disclosures: You have the right to request an accounting of non-routine disclosures of your personal health information. Submit your request in writing to the Privacy Officer.

 

Right to File a Complaint: If you believe your privacy rights have been violated, you have the right to file a complaint with our agency or the Secretary of the Department of Health and Human Services. Contact our Privacy Officer for more information.

Authorization for Certain Uses and Disclosures: Some uses and disclosures of your personal health information require your written authorization. You may revoke such authorizations at any time by submitting a written statement.
